What forms should I use?
- All construction contracts and Architect/Engineer forms should be obtained from the Ohio Facilities Construction Commission website, http://ofcc.ohio.gov/About.aspx
- Two year colleges should use the two year supplementary instructions and supplementary conditions from the Ohio Facilities Construction Commission website.
- The construction contracts and Architect/Engineer forms have been modified to change references from the State Architect’s Office to the Ohio Facilities Construction Commission.
- The construction contracts and Architect/Engineer forms have also been modified due to the elimination of the Declaration of Material Assistance Form, effective September 10, 2012.
- If a state client wishes to modify the forms that are published by the Ohio Facilities Construction Commission, they must file Supplementary Instructions and/or Supplementary Conditions with the modifications. All requests for filing Supplementary Instructions and/or Supplementary Conditions should be sent to the Ohio Facilities Construction Commission.
